This template is the official Capstone Competency Check List for each ASOT OTD student and their faculty supervisor to document the student's progression through essential ASOT OTD program course work before progressing to Level II Fieldwork, to post-Level II Fieldwork courses, and finally, to the OTD Capstone. Each item on this template must be successfully completed with a passing score before progressing to the next phase of the student's education in this OTD program.
