American Occupational Therapy Association Fieldwork Data Form
The AOTA Fieldwork Data Form is document that provides a comprehensive overview of the fieldwork agency, services provided, and the clients/participants who receive services at/from the agency. The information captured on this form is shared with students of occupational therapy from any OT/OTA program before and during any fieldwork experience.
Students will also use the information on this form to assist them with site selection during their fieldwork preparation processes. In addition, students' likelihood to progress to entry-level competency more quickly during Level II Fieldwork experiences is increased if they are provided with the agency's AOTA Fieldwork Data Form within or before the first week of fieldwork. This information helps students provide client-centered care by understanding and utilizing the fieldwork agency’s human and environmental resources to the students' fullest potential.
Lastly, ASOT must maintain specific information about each fieldwork agency with whom the program is partnered to ensure alignment and to communicate agency information to its students (ACOTE Standard, C.1.2.). To this end, all fieldwork students must review the fieldwork agency’s current AOTA Fieldwork Data Form prior to or during their first week of fieldwork.
If the fieldwork agency’s existing AOTA Fieldwork Data Form is out-of-date (approximately 2 years or older), is no longer applicable (substantial operational changes have occurred, no matter the date), or does not exist, a designated person at the fieldwork agency will work with this program's Academic Fieldwork Director, Juliet Steffe, to create, update, or modify the data form using the template provided on AOTA's website OR by clicking the attachment below. The fieldwork agency is responsible for saving a copy of the completed AOTA Fieldwork Data Form to be kept as a part of its own records, shared with this and other OT/OTA programs, and easily updated as needed. ASOT will save fieldwork agencies' data forms as they are shared with this program and made accessible to students of this program.
